CQC report explained · a nursing home, run by Only Care (Blackwell)
Blackwell Care Centre
Gloves Lane, Blackwell, Alfreton, DE55 5JJ
Rated Good: inspectors found the service performing well and meeting their expectations.
What inspectors found, December 2023
Rated Requires Improvement; inspectors found kind staff but serious problems with safety, staffing, medicines and management.
The inspection was unannounced and took place over 24 and 26 October 2023. Inspectors spoke with people, relatives, staff and the manager. They observed care and checked care records, medicines, staff training, recruitment and quality checks.
The home was not always safe. Inspectors found unsafe moving and handling, unreported accidents and medicine problems. There were not enough staff at night to meet people's needs safely. Staff skills and competence were not consistently checked.
People were not always involved in care planning or supported to communicate their choices. Some people did not receive meaningful activities or enough support to leave their rooms. The home was clean, food was generally well regarded, and people and relatives described staff as kind and caring.
The overall rating and all five question ratings are Requires Improvement. The previous rating was Outstanding, published in January 2019 under the previous provider. CQC found five breached regulations and will monitor an action plan.
Kind staff
People and relatives spoke positively about staff. Inspectors also saw kind and thoughtful interactions.
“We saw lots of kind and thoughtful interactions between people and staff.” from the report
Clean home
The home was clean and infection prevention procedures were being followed.
“The service was kept clean and people were protected from the risk of an acquired health infection.” from the report
Healthcare access
People could access external healthcare services, and staff contacted health professionals when they were concerned.
“People had access to GP, dentist services and other healthcare professionals.” from the report
Food choices
People generally viewed the quality and variety of the food positively. Options were available for specific dietary requirements.
“People told us the quality and variety of the food was generally good.” from the report
Night staffing
seriousThere were not enough staff at night to meet people's needs safely. Inspectors found this could put people at risk during routine care or an emergency.
“Due to a lack of staff at night, people were placed at risk of harm.” from the report
Unsafe care and incident reporting
seriousStaff did not always put their training into practice, and some accidents and medicine incidents were not reported. This reduced the provider's ability to learn from problems.
“Accidents and incidents were not always reported, which meant the provider was not consistently able to use this information to identify trends and to prevent reoccurrences.” from the report
Consent and capacity
seriousCapacity assessments did not consistently record people's views or explain the decision being considered. Staff did not consistently understand how to support people to make decisions.
“Records of assessments of capacity did not consistently document the views of people or relatives, or clearly state what decision the person was being asked to make.” from the report
Communication and activities
needs fixingPeople who needed alternative communication methods did not always receive them. Some people were not consistently supported with relevant activities, including regular opportunities to leave their room.
“People were not consistently supported to take part in activities that were relevant or meaningful to them.” from the report
Weak oversight
seriousThe provider's checks did not reliably find or correct problems, and earlier feedback had not consistently resulted in improvements.
“The provider's audit systems failed to identify issues we found on this inspection.” from the report

Or stay at home with a private carer
Many people who look at care homes end up staying at home with a carer who comes in for a few hours a day, or lives in. PrimeCarers introduces you to self-employed carers you choose yourself: interviewed, ID and DBS checked, from about £20 an hour or £1,120 a week live-in.
