CQC report explained · a nursing home, run by The Graham Care Group
Rodwell House
Brox Lane, Rowtown, Addlestone, KT15 1HH
Rated Requires improvement: inspectors found the service was not performing as well as it should and told it what to change.
- Safe?
- Good
- Inspectors found clear and up-to-date risk assessments, enough staff, safe medicine systems and appropriate safeguarding processes. Some people and relatives said waits for staff could still be quite long at busy times, and inspectors raised concerns about how sluice rooms were being used.
- Effective?
- Requires improvement
- Staff had training and people could access health professionals. However, some staff did not fully understand the Mental Capacity Act, and some meals were not hot enough, particularly for people eating in their rooms.
- Caring?
- Good
- People and relatives described staff as kind, patient and respectful. Inspectors saw staff protecting privacy, asking for consent and encouraging people to remain independent.
- Responsive?
- Requires improvement
- Care plans contained detailed information about people's needs and preferences, and complaints were investigated. However, activities were limited for some people, there was no daily activity plan, and some people reported communication problems.
- Well-led?
- Requires improvement
- Leadership and oversight had improved after a new registered manager was appointed. However, the home's checks had not identified that some preferences were not being followed, so improvements were not yet fully consistent.
What inspectors found, May 2023
Rodwell House is rated Requires Improvement; inspectors found safe and kind care, but activities, some meals, personal preferences and management needed more consistent attention.
This was an unannounced follow-up inspection on 12 April 2023. Inspectors spoke with people living at the home, relatives and staff. They observed care and reviewed care plans, medicine records, incident records, training files and quality checks.
The home had improved since the previous inspection. People said they felt safe and were treated kindly. Medicines, staffing, risk assessments, safeguarding and incident monitoring were generally managed safely. The environment had also been improved for people living with dementia.
There were still important gaps. There were not enough activity staff to meet everyone's social needs, and some people said they were bored. Some meals were not hot enough, some personal preferences were not followed, and there was confusion about mental capacity assessments and legal safeguards.
The overall rating remains Requires Improvement. Safe and Caring were rated Good. Effective, Responsive and Well-led were rated Requires Improvement. The home was no longer breaching regulations, but CQC will continue to monitor its progress.
Safer care systems
Risk assessments, care plans, medicines and incident records were generally managed safely. The provider had acted on the previous breach relating to risk knowledge.
“Risk assessments were clear, comprehensive, and up to date.” from the report
Kind and respectful staff
People and relatives said staff were kind and caring. Inspectors observed patience, reassurance, respect for privacy and support for independence.
“People told us staff were kind and caring and treated them respectfully.” from the report
Improved dementia-friendly environment
The home had made significant environmental improvements, including clearer signs, different-coloured doors and memory boxes to help people recognise their rooms.
“People's doors were different colours and memory boxes had been put up outside people's rooms.” from the report
More open leadership
People, relatives and staff reported improvements since the new registered manager started. Relatives were invited to meetings and received updates.
“Relatives said they now felt more informed for the first time, and they believed this was the start of something long term and positive.” from the report
Limited activities
needs fixingThere was only one activity coordinator and no daily activity plan. People who stayed in their rooms or needed care in bed had particularly limited choices and some said they were bored.
“For people who spent a lot of time in their rooms or needed to be cared for in bed, there were limited activity choices.” from the report
Meals not always hot
needs fixingSome meals, especially those delivered to people's rooms, were not hot enough because they were transported in a non-heated trolley.
“Some people told us their meals were still not always hot enough.” from the report
Preferences not always followed
needs fixingInspectors found examples where recorded preferences were not followed, including preferences about staff gender, food and showers. The home's checks had not identified these problems.
“Sometimes people's preferences were not adhered to, for example, one person who had requested only female staff, had care given by male staff on occasions.” from the report
Confusion about legal safeguards
needs fixingSome mental capacity assessments and best-interest decisions were not used consistently. Some staff did not fully understand the Mental Capacity Act and related applications.
“Although care was provided in the least restrictive way, there was some confusion around mental capacity assessments and DoLS applications.” from the report
Sluice rooms used incorrectly
minorInspectors were only somewhat assured about some infection-control arrangements because sluice rooms were not being used correctly. They gave feedback and recommended further guidance.
“Sluice rooms (dirty utility rooms) were not being used correctly.” from the report
- 01What daily activity timetable is now in place, and how will you provide meaningful activities for people who stay in their rooms or need care in bed?
- 02How do you keep meals hot for people who eat in their rooms, and how do you check that food choices meet their preferences?
- 03How do you check that recorded preferences, such as preferred staff gender, shower routines and disliked foods, are followed every day?
- 04What extra training or checks have been introduced to make sure staff understand mental capacity assessments and DoLS applications?
- 05What action have you taken about the incorrect use of sluice rooms, and when was this last checked?
This was an unannounced follow-up inspection of the care home and the personal care provided to people living in rooms under tenancy agreements; infection prevention and control was also reviewed. Or stay at home with a private carer
Many people who look at care homes end up staying at home with a carer who comes in for a few hours a day, or lives in. PrimeCarers introduces you to self-employed carers you choose yourself: interviewed, ID and DBS checked, from about £20 an hour or £1,120 a week live-in.
